Liability Release & Agreement

By completing this booking form, you ("Client") agree to the following terms and conditions with True Companions ("Service Provider"), operated by Zion Matthews in South Florida.

1. Assumption of Risk. Client acknowledges that dog walking involves inherent risks, including but not limited to: dog bites, escape, injury, illness, or death of the pet. Client voluntarily assumes all such risks on behalf of themselves and their pet(s).

2. Release of Liability. Client releases, waives, and forever discharges True Companions and its owner, agents, and employees from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury (including death) that may be sustained by Client's pet during the provision of services, except in cases of gross negligence or willful misconduct.

3. Health & Vaccination. Client certifies that the dog(s) are in good health, are current on all vaccinations (including rabies), are free of contagious diseases, and are not known to be aggressive toward people or other animals unless disclosed in writing to the Service Provider.

4. Emergency Authorization. In the event of a medical emergency, Client authorizes True Companions to seek veterinary care at Client's expense. True Companions will make every effort to contact Client and/or listed emergency contacts first.

5. Leash & Equipment. All dogs will be walked on a leash at all times. Client agrees that their dog has no history of severe aggression that has not been disclosed. If a dog displays aggression during a walk, the Service Provider reserves the right to end the session and notify Client.

6. Cancellation Policy. Cancellations made less than 24 hours before a scheduled walk are subject to a 50% cancellation fee. No-shows will be charged the full session fee.

7. Photo & Media Release. Client grants True Companions permission to photograph and video their pet(s) for use in social media and marketing materials, unless explicitly opted out of in writing.

8. Governing Law. This agreement shall be governed by the laws of the State of Florida. Any disputes shall be resolved in the appropriate court in Miami-Dade County, Florida.

I have read and agree to the Liability Release & Agreement above. I confirm that my dog(s) are current on all vaccinations and I have disclosed any known health conditions or aggressive behaviors. I authorize True Companions to seek emergency veterinary care if needed and agree to be responsible for any associated costs.

Cancellation policy: Cancellations made less than 24 hours before the scheduled walk are subject to a 50% fee. Same-day cancellations and no-shows will be charged the full service rate.
